Photography and Videography may take place during the CA for ALL Ages & Abilities conference. Footage and photos may be used for promotional, media, and public relations purposes. Please review the Consent and Release agreement.

Consent

I authorize the gathering, photographing and recording of my name, picture, likeness, voice, statements, signature, actions, testimonials, endorsements and/or biographical information (collectively, "Likeness") by or on behalf of CA for ALL Ages & Abilities, and irrevocably consent and grant to CA for ALL Ages & Abilities, its affiliates, agents and assigns (collectively, "CA for ALL Ages & Abilities") the right to use, edit and reuse my Likeness for purposes of advertising, media, public relations, or any other lawful use.

Ownership

I agree that CA for ALL Ages & Abilities shall be the exclusive owner of the results of such gathering, photography, and recordings, with the right to copyright, edit, alter, use and license others to use, in any manner or form, in any and all media whether now known or hereafter devised, throughout the world, all or any portion of my Likeness, or any reproduction or modification thereof. I waive any right that I may have to inspect or approve the copy and/or finished product or products that may be used in connection therewith or the use to which it may be applied.

Release

For myself and anyone acting on my behalf, and my heirs, executors and assigns, I irrevocably release, waive, discharge and hold harmless CA for ALL Ages & Abilities of and from any claims, obligation or liabilities of any kind or nature at any time, known or unknown, arising directly or indirectly out of or in connection with the use of my Likeness. I waive any rights I may have under California Civil Code § 1542, which provides: "A general release does not extend to claims that the creditor or releasing party does not know or suspect to exist in his or her favor at the time of executing the release and that, if known by him or her, would have materially affected his or her settlement with the debtor or released party."

General

This Consent and Release represents the entire agreement between us as to its subject matter, can only be modified in writing signed by both parties, and is governed by the laws of California. This Consent and Release may be executed and/or acknowledged by signing it and returning it to CA for ALL Ages & Abilities or it may be executed and/or acknowledged electronically. Any electronic symbol or process attached to or logically associated with this Consent and Release and adopted by a party with the intent to sign this Consent and Release shall have the same legal validity and enforceability as a manually executed signature to the fullest extent permitted by applicable law, including the Federal Electronic Signatures in Global and National Commerce Act and the California Uniform Electronic Transactions Act.

The novel coronavirus, COVID-19, has been declared a worldwide pandemic by the World Health Organization. COVID-19 is contagious and may be spread by person-to-person contact. CA for ALL Ages & Abilities event organizers, its partners, and affiliates are committed to protecting persons attending the in-person event and related activities, including members, staff, guests, vendors, and other support personnel. CA for ALL Ages & Abilities activities will be hosted in accordance with the health and safety policies as mandated by the state of California and the city of Sacramento at the time of the event.

Despite reasonable precautions taken, CA for ALL Ages & Abilities, its affiliates, and partners cannot ensure that attendees will not be exposed to or contract COVID-19 as a result of participating in this event or related activities.

By registering for this event or related activities, attendee acknowledges the highly contagious nature of COVID-19 and voluntarily assumes the risk for exposure or infection by attending, and that such exposure of infection may result in personal injury, illness, disability, and/or death to attendee. Attendee understands that the risk of becoming exposed to or infected by COVID-19 at the event or related activities may result from actions, omissions, or negligence of others who may attend or their families, colleagues, or others with whom they may have contact. Accordingly, attendee understands and agrees to hold harmless CA for ALL Ages & Abilities event organizers, partners, affiliates, and attendees, whether a COVID-19 infection occurs before, during, or after participation in the event.

Attendee agrees to comply with all COVID-related procedures that may be implemented by CA for ALL Ages & Abilities event organizers, partners, affiliates, and the event venue, including but not limited to, mask-wearing, physical distancing, and restrictions on certain activities that carry higher COVID-19 related risks, in order to protect, as much as possible, the health and safety of all participants.
